Whether at home or in a medical spa, you need to exfoliate.

Abby Borbon-Roaquin, manager and clinician at Ulan Medical Spa in San Diego, California, has been a registered nurse for over 14 years, the last four of them in aesthetic medicine. Here, she explains the process of exfoliation and how it is done both in medical spas and at home.

Why You Need to Exfoliate

Exfoliation is a natural process through which your dead skin cells are replaced, exposing the new skin underneath. Normally, it happens about every five weeks. But as we age, our skin doesn’t exfoliate as quickly as it did when we were younger, so we need a little boost. When you don’t exfoliate, you get a buildup of dead skin, which makes it look dull and gives it an uneven texture. Exfoliation smoothes the skin’s texture and promotes skin turnover. Dead skin build-up can accentuate lines and wrinkles, clog your pores, and make you more prone to acne.

Medical Spa Exfoliation Treatments

Skin can be exfoliated mechanically or chemically:

1. An example of mechanical exfoliation is microdermabrasion. A quick procedure designed to remove dead skin cells, microdermabrasion uses either fine aluminum oxide crystals or a diamond-tipped head to gently abrade the skin as the tip planes across with a slight vacuuming action.

2. An example of chemical exfoliation is chemical peels. A chemical peel is a procedure that uses a chemical solution that causes the outer layers of damaged skin to peel off. This treatment can be customized according to your goals and needs by selecting the type of acid used, the amount of time your skin is exposed to it, and the number of layers applied. For example, glycolic acids and trichloroacetic acids are usually used for anti-aging and pigment problems, while salicylic acids are usually the choice for acne-prone skin.

At-Home Exfoliation Treatments

At-home exfoliation is a must in order to maintain and enhance the results achieved through in-office treatments. Face washes, gels, and lotions that contain chemical exfoliants can be used at home, as the concentrations are designed for daily use.

At Ulan Medical Spa, we recommend a face wash that contains 7% glycolic acid by Prescribed Solutions, a physician-grade skincare line. We may also recommend a mini-peel gel by Glytone, also physician-grade line. This mini-peel gel, which contains 10.8% glycolic acid, is applied to the face. You leave it on for about 10 minutes, two to three times a week, at home to give your skin a boost. You may have to ease into this regimen. For example, especially if you have sensitive or dry skin, you could start twice a week with the face wash, and work your way up to being able to do it daily.

Deciding to have plastic surgery is an enormous step, and not one to be taken lightly. They key to optimal results is deciding on the right procedure for you, making sure you are an appropriate candidate for it, having realistic expectations from the very start, and being aware of the recovery period and correct after-care needed to make the most of your experience.

Just make sure you're doing it for all the right reasons...

Too many people decide to have cosmetic or plastic surgery for the wrong reasons: to make someone else (such as a husband or boss) happier, change them into a radically different person altogether, to solve domestic problems, give in to peer pressure or heal grief. But plastic surgery is designed with one goal in mind: to change a person’s appearance, hopefully for the better, not as a surgical solution to life’s many problems.

If you are considering having plastic surgery, the world’s your oyster. There are literally hundreds of procedures out there to suit everyone, from breast augmentation to breast reduction, from thigh liposuction to calf implants. Whether you want your nose made smaller, your lips made bigger or your tummy flatter, all you need to do is decide what you want, find a good surgeon, then do it. Just make sure you’re doing it for all the right reasons…

Most Popular Procedures

The American Society for Aesthetic Plastic Surgery (ASAPS) says that a whopping 10 million surgical and non-surgical cosmetic procedures were carried out in the US in 2008, raking in $12 billion worth of business. Since 1997 all procedures are up 65 percent, with non-surgical ones topping the charts with a 239 percent increase, thanks mainly due to the popularity of Botox.

Top Five Most Popular Surgical Procedures (according to the ASAPS, for both men and women, in 2008)

1 Breast augmentation (average cost: saline $3,603; silicone $3,885 )

2 Liposuction (average cost: suction-assisted: $2,874; ultrasound-assisted: $2,933 )

3 Eyelid surgery (average cost: $2,921)

4 Rhinoplasty, or nose job (average cost: $4,369)

5 Abdominoplasty, or tummy tuck (average cost: $5,470)

Top Five Most Popular Non- Surgical Procedures

1 Botox (average cost: $443)

2 Laser hair removal (average cost: $326)

3 Hyaluronic acid, including Juvederm, Restylane/Perlane, Hylaform (average cost: $411 – $657)

4 Chemical peel (average cost: $704)

5 Laser skin resurfacing (average cost: $2,094)

Playing It Safe

Now that you have a procedure in mind, you need a decent surgeon. The American Society of Plastic Surgeons lists certain guidelines when it comes to selecting both the right surgeon and the right procedure for you. It recommends asking the following questions when deciding who’s the right man (or woman) for the job:

1. Are you an ASPS Member Surgeon?

2. Are you board-certified by The American Board of Plastic Surgery?

3. Do you have hospital privileges to perform this procedure? If so, can you list the hospitals?

4. Is the surgical facility you regularly use an accredited one?

5. Can you tell me how many procedures of this type you have performed in the past?

It also recommends these questions to ask the surgeon you potentially choose, to learn more about their credentials, safety record and experience:

Am I a good candidate for this procedure? If you want a face lift yet you haven’t yet reached puberty, a good surgeon will refuse.

What will be expected of me to get optimal results? You may need to lose weight, come off certain medications, stop smoking etc. A good surgeon will make sure you are fit for surgery, not speed the process along to make more money.

What are the risks involved with my procedure? Risks of any surgical procedure are rare, but they do exist. A good surgeon will make you fully aware of what you are getting into well in advance, even if that means potentially scaring off a few potential patients.

How long of a recovery period can I expect, and what kind of help will I need during my recovery? Make sure you have the right support network you need and that you are fully aware of all after-care procedures for best results.

Will I need to take time off work? If so, how long? It’s no use having surgery and then getting an infection straight afterwards because you jumped back into work too soon.

You should also ask your surgeon about their revision policy, in case something goes horribly wrong. Do they waive the fee, or offer a cut-price rate? Even if they do waive it, would you still have to shell out for hospital fees and anesthesia? Or do you want to spend the rest of your life with one nostril noticeably larger than the other, or with debilitating pain and even paralysis following a course of botched liposuction?

Risks vs Rewards

Having plastic surgery is a much more complicated decision than deciding which restaurant to visit for dinner or where to go on your next vacation. Before you decide on such a monumental, life-changing course of action, make sure you explore all the many facets such a huge step will entail, physically, emotionally and (oh yes) financially. In fact, after thinking long and hard about it, you may decide that going under the knife for purely cosmetic reasons isn’t really for you after all.

“If you want to get cosmetic surgery, get real with yourself first. Get in touch with your authentic self so that you know who you truly are and what you truly want,” says Dr Phil on his website.

“The only person who can give you what you want is you. Nobody else can give you the self-esteem you need to have a healthy body image. A surgeon can’t. The only thing that can ‘fix’ you for you comes from the inside — not the outside.

“There is no reality, only perception. If your self-esteem is so low that you view yourself through a filter of self-rejection (’I'm ugly’ or ‘I’m fat’), all the surgery in the world isn’t going to make a difference. If you don’t change the filter through which you look when you see yourself in the mirror, you’ll never be happy.”

You deserve to be scar-free, and today’s medical technology provides many options for making that happen.

Are you harboring a scar in a noticeable place, and is it making you self-conscious? Do you find yourself feeling the need to cover up, maybe wearing long sleeves in the summer or extra concealer on your face? You shouldn’t have to be ashamed of anything when you leave your house, and you definitely shouldn’t have to cover anything up. An injury or a surgery shouldn’t have you worrying about hiding anything. Not to mention your scar may be painful, a situation that definitely calls for removal as soon as possible. You deserve to be scar-free, and today’s medical technology provides many options for making that happen. Scar removal used to be a painful, invasive, and expensive procedure, and in many cases, the scar was not bad enough to warrant such a process. But with the options available now, any scar can be made history.

What Kinds of Scar Removal are Available?

All the types of scar removal you hear about are done by your dermatologist. Your dermatologist will help you decide which option is the best for you and your scar. One of the most popular scar removal processes is laser scar removal. In this procedure, your dermatologist uses a high energy light to “remove or reshape areas of the skin that are affected by scar tissue.” There are even different types of procedures within this laser treatment category. Raised scars or reddened scars can be treated with a pulsed dye laser, and scars that are left by acne can be treated with laser skin resurfacing, during which a laser is used to “dissolve the molecular bonds of the damaged skin cells layer by layer until a smoother, more uniform skin appearance is achieved.”

Laser scar removal, or laser skin resurfacing, is the most expansive, effective removal option.

Microdermabrasion is a more cosmetic, less medical option. Completely non-invasive, this procedure involves spraying microcrystals onto the skin to remove the outermost layer of dead skin cells, leaving behind the younger, healthier layer of skin. Furthermore, Microdermabrasion is a non-chemical, non-invasive procedure that uses a spray of microcrystals to remove the outermost layer of dry, dead skin cells and reveal younger, healthier-looking skin. Microdermabrasion also encourages the production of a new underlying layer of skin cells with higher levels of collagen and elastin, which further improves your skin’s appearance.

A chemical peel is one of the older options – well, to be completely correct, it’s definitely the oldest since it was performed in ancient Greece, Rome, and Egypt. It involves applying an acid solution to your skin and peeling off the top, dead layer, to leave a fresh layer without deep wrinkles, uneven coloring, and, most importantly, scars.

Who Should Get Which Type of Removal Procedure?

Of course, as we mentioned, your dermatologist will help you figure out which process is best for you. Now, a lot of websites for scar removal list collagen injections as a removal option, but when you read up on it, it becomes clear that collagen injections are more what you probably thought they’re for: wrinkles. Collagen works to fill in deep lines. Now, that basic principle can apply to filling in an indented scar, but really seems to be more of a cosmetic, anti-aging procedure.

Laser scar removal, or laser skin resurfacing, is the most expansive, effective removal option. This will probably be the choice for you if your scar is deeper, more serious, and/or painful, since it is not just a cosmetic procedure. While this is a non-invasive procedure as much as any laperscopic procedure is, it is of course more invasive than a cosmetic option like microdermabrasion. Laser scar removal can treat anything from discoloration, wrinkles and sun damage to acne marks and other types of imperfections. In severe cases, of course, more than one procedure may be necessary, but the laser option is the best way to go for more serious scarring.

Microdermabrasion is the option for less serious scarring, and is geared toward anyone with some lighter scarring to anyone who has discoloring, wrinkles, and/or blemishes. It’s described as the procedure for those who don’t have the time or resources for the more invasive choices like laser scar removal, and so obviously this procedure is for someone who does not have as dire of a need for removal, such as someone who is not experiencing pain from his or her scar, as this process does not extend as far into the skin for removal.

The chemical peel, interestingly, actually has a specific type of candidate it works best for, which is a person with fair coloring and fair hair. This is because its results are the best and most effective with uneven coloring, or pigmentation, and the chemical peel will often leave behind the lighter color of skin. So, if your scar is noticeable because it is a different color, the chemical peel may very well be the best choice for you. It’s also key that the chemical peel candidate does not smoke.

What are the Benefits/ Risks of Each Choice?

The benefits of laser scar removal are most importantly effective scar removal, but also refreshed, younger looking skin; improved, softer texture; sun damage repair; and even an early treatment/prevention for skin cancer.
The risks include the rare case of infection, and, surprisingly, those who are prone to scarring after minor injury, as even a scar removal process can create new scarring on those who are prone. Those with darker skin can experience permanent darkened spots – some of these are substantial risks to consider.

The benefits of microdermabrasion include the fact that it is a “lunch hour” procedure, meaning it can be done in 30-60 minutes with no down time for recovery, and it is totally non-invasive. It removes light scarring and freshens skin, removing wrinkles and blemishes. It has virtually no risks or side effects, with the exception of the chance of some slight irritation.

The benefits of the chemical peel are similar to microdermabrasion, including removal of sun damage, wrinkles, blemishes, and most importantly, scarring. A chemical peel can usually handle scarring more serious than microdermabrasion, but less so than laser scar removal can. A great benefit is that there are varying levels of a chemical peel, light, medium, and deep, so as to treat your own personal issues as best as possible. The risks are rare, but include the slight chance of scarring, infection, swelling, and cold sore outbreaks – yikes! But following a doctor’s instructions for post-peel care can reduce those risks a lot. Remember, your dermatologist will guide you in the process of choosing which option is best for you.

A chemical peel, a procedure in which an acid solution is utilized to remove the top layers of skin, can restore a more youthful appearance to skin that is wrinkled, discolored, mildly scarred or sun-damaged. When the dead or damaged layers of skin are removed, younger, more vibrant skin is revealed. Chemical peels can significantly reduce the appearance of acne scars and improve the skin’s overall texture, smoothing fine lines and wrinkles and correcting discolorations such as age spots.

Chemical peels are generally safe and can be performed quickly, getting you back to work with just a minimum of recovery time. A light chemical peel can be completed in just 15 minutes and requires virtually no time to recover. Deeper chemical peels may require a slightly longer recovery time, up to about a week. Chemical peels are associated with few risks, and most of them are relatively minor. Because these peels have been used successfully for many years, there has been ample time to perfect the procedure and to minimize risks and discomforts.

A chemical peel can be an affordable alternative to more expensive cosmetic procedures. Ranging in price from $150 for a very light peel to $2000 for a deep peel, chemical peels can offer dramatic results at a price that is accessible for most women.

Many people think of a med spa as a type of one-stop shopping for all their personal cosmetic needs. In theory, a med spa – short for medical spa – is a cross between a doctor’s office and a day spa, with all procedures carried out under the supervision and authority of a licensed medical doctor. The reality, however, is far from that.

Not all medical spas cater to the same clientele.

If you want to have a facial using the latest French products or a full back massage, then looking in the phone book and finding your nearest med spa is your best bet. But if you want anything more invasive, from having a laser hair treatment to trying out the latest chemical peel, it’s best to do your homework first.

Better Safe than Sorry

Visiting a med spa should be fun, relaxing and productive, and not result in a visit to the emergency room, infection or permanent scarring. While the non-invasive treatments carried out at most med spas have less risk of complication than full-on plastic or cosmetic surgery, serious injury could still occur. Ask these questions before you book an appointment:

Is there a doctor in the house? Med spa regulations vary from state to state. While medical treatments are, in theory, supposed to be carried out only with full medical supervision, often the doctor is not even on-site, let alone in the same room. And in some cases, unlicensed personnel with only the most rudimentary training in a specific procedure will be working on you. Make sure there is a licensed full-time medical director and nurse on-site – preferably in the room with you, or at least in a supervisory position overseeing qualified medical personnel. It’s advised that the doctor be either a licensed plastic surgeon or member of an affiliate group such as the American Society of Aesthetic Medicine.

Is staff experienced in specific procedures? A med spa may have ten years of experience overall – but only two weeks’ experience in the procedure you want to have carried out. Find out who your practitioner will be and ask how many times he or she has carried out the specific procedure you want – in the last year, month and week. Also, find out how often serious side effects occur – this should happen with less than one percent of the treatments provided. Finally, check credentials and ask about training and background. If staff appear insulted by your questions, go somewhere else.

Is the consultation up to standard? It’s no use visiting a med spa where personnel are condescending to their clients and questions are either brushed off or not allowed. Real med spas encourage their patients to ask as many questions as they need to until they feel entirely comfortable, and will give them up-to-date information to take home and leaf through in private. Consultations should be one-on-one with a medical professional, not a secretary, and should leave you feeling satisfied with the outcome, not uncomfortable or confused.

What is the equipment like? The equipment should not only be up-to-date, well-maintained and sterile, but there should be the right choice available for different skin types – different types of FDA-approved lasers, for example. There also be resuscitation equipment on hand, especially if the spa offers procedures such as varicose vein removal and deep chemical peels. The equipment should not only be clean but the entire facility should be hygienic, with proper hand-sterilizing facilities available for both guests and personnel. If the facility cannot afford to invest in quality equipment, you can’t afford to invest in them.

Are clients satisfied overall? Find out if patients return for repeat visits because they are so satisfied with the med spa’s reputation and practice, or if they vow never to come back again. Some med spas draw people in with seemingly cheap prices, but for some reason nobody ever returns. Find out why. If you are interested in one specific procedure, ask if the med spa can provide contact details of satisfied patients. A confident doctor would not mind doing this – or will have patients contact you.

Is the price right? Beware the bogus scams. If it sounds too good to be true, it probably is – and if the price is way out of your reach, what’s the point? Med spa websites that make impossible promises or offer prices that are significantly cheaper than other places in the same area are usually not to be trusted. And remember, over-eager med spa personnel who want to sell you a series of treatments usually don’t have your best interest at heart (unless you’re impossibly ugly, that is). In most cases, their main goal is to make money.

Do’s and Don’ts in Choosing the Right Med Spa

Now that you know the basics, you may think it’s all plain sailing from here on in. But many people still get roped in by unbelievable promises, and make decisions they later regret. Here are a few do’s and don’ts to follow, to make sure you get the best treatment at the right price…

DO take your time in finding a med spa that’s right for you, where you feel comfortable, and where the staff are fully trained, certified and licensed. Time spent now will save you hours rectifying mistakes later.

DON’T go for the lowest-price option. If a price is way too low, that usually means something fishy is going on. You’re talking about your body, not just buying a new dress or fancy pair of shoes. You really can’t afford to risk your health, can you?

DO make sure the clinic or facility in question is devoted to med-spa procedures and aesthetics in general. You don’t want to be seen by a chiropractor who is making a bit of money on the side by running a third-rate med spa. Or by a proctologist with no training in dermatology who is making money by doing facial skin procedures.

DON’T be embarrassed to leave if something doesn’t feel right. Often, our gut instinct is all we have. Rely on it. If something seems more than a little suspicious or a bit out of kilter, there’s probably a damn good reason. Go with your intuition and go somewhere else.

DO make sure that all medical procedures are performed by medical personnel, not unlicensed or non-medical staff. Check state laws to see if procedures such as giving injections and doing deep chemical peels must be preformed by a doctor, or if nursing staff can carry them out under a doctor’s supervision. And if you see an untrained secretary giving injections or doing a chemical peel, don’t be afraid to report them. You may be doing someone else in future a big, big favor.

DON’T go to someone’s home, a hotel room or a temporary rented space for a medically related treatment. You would never have your tonsils taken out on your doctor’s kitchen table, would you? Similarly, be wary of locations in shopping mails or adjacent to nail salons that are outside a proper medical facility. Even if they are legit, there may be no resources available if something goes horribly wrong. The American Society of Plastic Surgeons as well as the American Society for Aesthetic Plastic Surgery recommends only visiting a med spa located within a physician’s office.

Choosing the right med spa, in the vast majority of cases, is a bit trickier than choosing the right place to have a haircut – and the consequences of choosing the wrong one can be much more dire than a broken fingernail. Do your homework carefully and make sure you make the right choice. Your health, safety and permanent appearance may depend on the choices you make.

Small children have incredibly beautiful skin. Not only is it fresh and free of age spots and wrinkles, it also has an amazing elasticity and appears to defy gravity. So why is it that when the gorgeous, fresh-faced child hits 30, 40 or 50, his or her skin will have taken on a completely different appearance?

Beautiful skin affects your mood as well.

Like all the organs in our body, our skin ages with time. In fact, it often appears to age faster than other parts of the body, as much of our skin – especially on the face – is regularly exposed to the sun and a host of other environmental factors such as pollution, which accelerate the aging process.

The sun not only causes us to wrinkle, but also can result in age spots, freckles, blemishes and even cancer. It can make a 35-year-old appear 60, if he or she regularly exposes their skin without taking any precautions against the sun’s harmful rays.

While it’s impossible to look 30 years younger – and also bit stupid to even try – there are ways to make your skin look healthy, refreshed and considerably younger than it actually is. If you want to turn back the clock on your skin, don’t invest in miracle anti-aging products or med spa procedures – at least, not until you change your lifestyle habits first. Once you have a healthy routine in order, you might want to try other methods…

Chronological Aging

Our skin ages from the inside as well as the outside – called intrinsic aging – simply due to the fact that we are getting older with each passing second. As our skin ages, we are more likely to:

Develop tumors, both malignant and benign, as well as other “blemishes” such as age spots, liver spots, seborrheic keratoses etc.

See a proliferation of abnormal blood vessels, particularly on the face, cheeks and ankles. Some of this is genetic, some is due to lifestyle.

Have a drier and thinner appearance of the skin in general. This happens as the skin takes longer to renew itself with age.

Witness less collagen being produced, which can lead to our cheeks appearing more hollow and other areas of the body looking less-plump. It will also cause sagging and the appearance of both deep and fine lines and wrinkles. It’s estimated that after age 20 we already see a one percent decrease in the amount of collagen we produce – every single year.

Accelerating the Aging Process

Often, we either wittingly or unwittingly do things which speed up the aging process on our skin, causing it to age extrinsically. In effect, we indulge in activities which age us quicker than normal, and often the first victim is our face. Such activities include:

Going to tanning salons. Tanning can cause basal and squamous skin cancers (see below), and while a tan may look good in the short run, it can make you look a lot older than you really are long-term – and be dangerous to your health.

Smoking. Free radical form in your body when you smoke. They are unstable molecules which can damage your DNA, making the cells behave erratically and therefore age faster. “Smoker’s face” is not a myth, and it affects women more than men. The expressions you make when you smoke – puckering and squinting – are common signs of classic smoker’s face. Nice.

Indulging in too much sun exposure. This exposes our skin to dangerous ultra-violet rays, which makes both collagen and the skin’s elastin begin to oxidize. Two main types of rays are UVA and UVB rays; the former can cause our skin to wrinkle as well as potentially fatal melanomas, while the latter can cause basal and squamous skin cancers. Spending your next two-week vacation baking on the beach is probably NOT a good idea.

Non-Invasive Ways to Slow Down Aging

Slow down the aging process by following these obvious, healthy steps:

Drink in moderation. Excess will only expand your blood vessels, causing a red face and itchiness. And who wants a grogblossom nose?

Quit Smoking. See above. If you need help to quit, look in the Yellow Pages.

Wear sunscreen. Sunscreen with a minimum of 15 SPF will protect your face from the photoaging effects of the sun. Some dermatologists recommend wearing a minimum of 35 SPF 365 days of the year, even inside.

Invest in good moisturizers. Look for ones that contain glycolic acid (alpahydroxy acids) and either retinoid or retinol, which have been proven helpful in reducing wrinkles and building up both collagen and elastin. Also look for ones with anti-oxidants, which can reverse free-radical sun damage. Examples are grape seed oil, Vitaimins E and C, green tea extract.

Eat sensibly. Good nutrition is important, and along with plenty of fresh fruits and vegetables and wholegrains, you should also have a diet with enough calcium, trace mineral and Vitamin D. Drinking lots of water is also important.

Rest/sleep. Your skin rejuvenates itself when you sleep, so make sure you get plenty of zzzz’s.

Stopping the Aging Process Through Invasive Means

While it is impossible to completely revere the signs of aging, there are certainly ways to make the damage that has already been done less visible – if you have a lot of cash, that is. They include:

Plastic surgery. From face lifts to thread lifts, a variety of procedures are available to improve the appearance of youth. Think long and hard if this is the right step for you, and only use a Board-certified surgeon with years of experience and hundreds of satisfied customers.

Botox. It’s estimated that more than two and half million Americans had Botox in 2008, to erase fine lines and wrinkles from the face. If having yourself injected with poison appeals, why not?

Chemical peels. Can improve the texture of skin damaged by sun exposure and other factors by removing the top layers with chemicals.

Nobody enjoys the thought of getting older, but the only alternative is death. If you want to maintain a younger, fresher appearance but don’t want to look like Joan Rivers, then stop smoking, have a healthy diet, exercise regularly, drink only in moderation and always apply sunscreen.

Genetics play an enormous role in how we look, but you can always make yourself look better by staying happy and healthy. And remember, you’re only as old as you feel. If you look fantastic and feel like crap, you must be doing something wrong (or perhaps something right?).

While the type of skin we have is determined largely by genetics, it's up to you to make sure that you keep it in top form. Eating well, exercising regularly and enjoying a healthy lifestyle while using plenty of sunscreen is important, as are visits to the dermatologist when warranted. It's all up to you!

All of us have minor problems with our skin from time to time, ranging from acne to irritated insect bites to itchy rashes that just won’t seem to go away. While the majority of minor skin complaints don’t warrant treatment, there are times when seeing a dermatologist is important, if not mandatory.

This is melanoma, or black mole cancer. If you have a lesion or mole that looks suspicious, see your dermatologist straightaway.

But how do you know when to just wait it out – or when to make an appointment?

A basic consideration to keep in mind is how long you have had the specific ailment. If you have a rash that doesn’t seem to get better within a few days, you might choose to see your primary care provider. If you still have no joy, this might signal the need to see a specialist. Similarly, a problem that keeps recurring may also make a visit to a specialist necessary.

Keeping in mind your own genetic history is important as well (see below). People with a history of skin cancer in their family, who are fair-skinned or prone to having a proliferation of moles on their face or body should also make regular appointments with their dermatologist to make sure they stay healthy.

What is a dermatologist?

A dermatologist is someone who knows everything there is to know about skin care. He or she will not only be able to diagnose your problem, but can also prescribe creams, medicines and treatment to either cure it, or keep it under control.

There are many types of dermatologists who can treat conditions of the skin, hair and nails, and who have specific specialties in areas such as cosmetic dermatology, pediatric dermatology etc. Whether you need medical, surgical or cosmetic treatment, they should be able to help you.

According to the American Board of Dermatology, dermatologists can help with the following conditions:

Diagnosis and treatment of all types of skin cancers, melanomas, moles, and other skin tumors.

Management of specific inflammatory skin disorders such as contact dermatitis, when the skin reacts after being exposed to specific allergens.

Recognition of the skin manifestations of certain infectious and systemic diseases.

Dermatopathology, or diagnosis of skin diseases, such as infections, immunologic and infectious diseases.

Surgical techniques used in dermatology, such as correction of acne scars, chemical peeling and laser surgery.

Cosmetic disorders, including hair loss, skin, and aging-related disorders.

Why See a Dermatologist?

Reasons to see a dermatologist include:

You suspect you may have skin cancer. Everyone should perform regular self-examinations of their bodies and be aware of what their moles look like. If you have a lesion or mole that looks suspicious, see your dermatologist straightaway. Moles that look unusual, have irregular borders or appear asymmetrical, have grown or bleed but do not crust over should be seen to immediately. Remember, with early detection skin cancer is often cured.

You have risk factors for skin cancer. Some people are at high risk for developing skin cancer, and along with regular self-examinations you should also see a dermatologist regularly to make sure their health is not at risk. Factors include personal history of skin cancer; close relative with a melanoma; fair skin that tends to burn or freckle; history of bad sunburns or tanning salon use; more than 50 moles on face or body; having moles that look irregular, large, or asymmetrical (see above); past use of x-ray treatments for acne, and taking medications to prevent arthritis or organ rejection.

You have a skin problem that just won’t go away. Many people choose to treat minor skin problems on their own. But if you have a rash that looks suspicious or does not respond to conventional treatment, it’s probably time to see a dermatologist. And if you successfully treat a skin complaint with an over-the-counter cream but it keeps coming back, it’s also a good idea to see a professional.

Your chronic skin condition that doesn’t respond to treatment. If you have eczema, for example, you may be able to keep it under control with moisturizers, emollients or over-the-counter steroid creams. But if you find your skin does not respond well, or if indeed the eczema gets worse of becomes infected, then seeing a dermatologist is your best bet.

You have a cosmetic skin problem which requires specialist treatment. If you have a skin problem that you think would benefit from medical intervention, seek professional help. It’s not advised to visit a med spa unless you are 100 percent positive that a dermatologist will be treating you. A licensed dermatologist is a physician certified by the American Board of Dermatology, and they have far more medical experience than cosmetologists and hair technicians!

What is Dermatologic Surgery?
Sometimes surgery is the only way to treat specific problems or cosmetic complaints. The purpose of dermatologic surgery is to “repair and/or improve the function and cosmetic appearance of skin tissue,” according to the American Society for Dermatologic Surgery. It “deals with the diagnosis and treatment of medically necessary and cosmetic conditions of the skin, hair, nails, veins, mucous membranes and adjacent tissues by various surgical, reconstructive, cosmetic and non-surgical methods.”

Dermatologic surgery today is a lot less invasive than it used to be, according to the Society, with less downtime and often no need for general anesthesia. This, it says, is largely thanks to breakthroughs in medical science, clinical advances and instrumentation innovations. Examples of dermatologic surgery today include:

Dealing with aging-related issues, such as brown spots, lines and wrinkles

Using injectable soft-tissue fillers

Using implantable soft-tissue fillers

Chemical peels

Liposuction

Laser surgery

Treatment for skin caner

Treatment for acne scars

Treatment for moles, rashes etc

Vein therapy (varicose veins and spider veins)

Hair restoration techniques

Reconstructive surgery

Our skin is the largest organ in your body, and taking care of it properly is important for our overall health, as well as our outward appearance. Our skin is our first line of defense when it comes to our immune systems, and regular care and maintenance helps us to ensure that we give ourselves the best chance we can of fighting off infection and staying healthy.

While the type of skin we have is determined largely by genetics, it’s up to you to make sure that you keep it in top form. Eating well, exercising regularly and enjoying a healthy lifestyle while using plenty of sunscreen is important, as are visits to the dermatologist when warranted. It’s all up to you!

Many of us choose med spas in an attempt to make us look younger and more attractive. One way to accomplish this is through skin rejuvenation treatments, which promise to make our skin look as new and fresh as a baby’s bum-bum. Chemical peels and mud treatments are among the most common – and the most popular – of these types of treatments. But few of us really know much about them, except for the cost and how long we’ll be expected to lie down with our eyes shut and/or our mouths closed. What exactly is the science behind them?

All About Chemical Peels

Some phenol peels are so powerful they can remove freckles.

A chemical peel, aka chemexfoliation or derma-peeling, is exactly what the name implies: chemicals are put on the skin to make its outer layer peel off, or shed, thus revealing fresher, new skin underneath. The thought behind this treatment is that age, the sun, stress, genetics and the environment, not to mention a host of other factors, combine together to wreak havoc on our skin, which is most evident in its outermost layer. So why not just peel it away?

Peeling the top layer of our skin away can make us look younger, more refreshed and more rejuvenated – even though the thought is more than a bit revolting. Having one done is not all fun and games, and all peels all carry some risk as well. They should only be carried out by a qualified plastic surgeon, not by some untrained med spa employee or a lab geek wielding a beaker of chemicals. While most peels are performed on the face, they can also be done on the neck and hands.

You will also need to follow all aftercare instructions very carefully, including staying out of the sun for quite a while afterwards, applying good quality sun cream on your face, avoiding the application of specific cosmetics and creams etc. Be aware also that after-effects are common with many peels, such as redness, stinging and flaking, but they should subside in time.

Types of Chemical Peels

Three main types of chemical peels exist, including…

LIGHT PEEL: Alphahydroxy Acid (AHA) Peels.AHAs are naturally occurring acids and, bizarrely, they are found in such substances as tomato juice and sour milk. Basically, they irritate the skin to such a degree that the skin begins to peel off. Although a AHA peel is considered the lightest of all peels, for some reason it often causes the largest number of side-effects, including redness and stinging, especially ones that contain beta hydroxy acid (BHA). It might be a good idea to treat a small patch of skin first and wait a few days to see how you react. AHA peels are the mildest and lightest type of peel for people who either have less severe skin problems or less time to have a more invasive peel (and people who are scared of anything more stronger). Types of acids used are not as strong as in other peels, and include gycolic, lactic and even fruit peels. Good to smooth rough skin, gets rid of dryness, acne and minor wrinkles.

MEDIUM PEEL: Trichloroacetic Acid (TCA) Peels. For medium depth peeling. The results last for a shorter period of time and are a lot less dramatic than with a phenol peel. The concentration of the actual chemical used ranges from 20 to 50 percent depending on what the practitioner decides is best for your skin. The chemical then penetrate all the way to the reticular dermis, made up of loose connective tissue. Better for people with darker skin, a TCA peel can correct blemishes, skin pigmentation problems and minor wrinkles.

DEEPEST PEEL: Phenol Peels. Strongest of all, good for patients who have suffered maximum skin damage or who have pre-cancerous growths on their faces. The beauty part is that an improvement can be seen after just one treatment, and it can last for up to two decades. Amazing! Uses carbolic acid to penetrate as deep as is safely possible into the skin, getting rid of deeper wrinkles and helping areas of the face that are sun-damaged or have skin pigmentation problems look better. Interestingly, this peel is so strong it will also remove freckles – keep this in mind if you have a freckled body – might look funny to have a plain face! Also, keep in mind that after the initial redness resulting form this peel subsides that your face will look whiter – not great if you have naturally dark skin (unless you want to be a Michael Jackson lookalike, that is!)

People have chemical peels for a variety of reasons. They can get rid of:

Age spots

Blemishes

Uneven skin pigmentation

Dull complexion

The appearance of wrinkles

Acne scars

Pre-cancerous growths

Be aware that chemical peels cannot work miracles, such as removing wrinkles completely, or getting rid of deep sags or bulges in the face. For that you would need to have laser resurfacing or plastic surgery.

Magical Mud
In the bad old days of the Soviet Empire, women relied on beauty basics to keep themselves looking great. Public bath-houses were often full of women scrubbing and rubbing themselves clean not with expensive French beauty products, but with jars of old coffee grounds, pots of honey and cartons of yogurt or curdled milk. They exfoliated, softened and smoothed with these all-natural products, which cost next to nothing, and never felt better.

Now Western women are visiting med spas to recreate these exfoliating and softening experiences not with household groceries but with magical mud – to not only make their skin soft and smooth but also to relieve stress, aches and pains, and even some painful medical conditions. Pelotherapy, or using natural mud or clay for therapeutic purposes, has been around for thousands of years, involving anything from applying a mud mask to slathering yourself in mid from head to toe.

Mud is a much less invasive way to make your skin look better compared to a chemical peel, through either facial mud packs or complete body immersion, and is often used a treatment for both younger and older women who simply want softer skin. Mud itself largely consists of clay, minerals and water, and the thought is that together they are seen as a gift from the earth when used expressly for rejuvenation and health purposes. The mud is often mixed with essential oils and slightly heated before application, and the majority of people enjoy the sensation.

Mud works on our skin because when it is mixed with moisture, it has the capacity to draw out impurities, thus helping us to cleanse, purify and refresh. It can also draw substances inside its own molecular structure and hold them there, thus trapping toxins within it. Composed of chemical compounds which are naturally rich in aluminum and silica, it also may contain other trace minerals such as zinc, copper and magnesium.

There are four main types of mud used in med spas today:

Moor mud. Thought to contain over 1,000 natural chemicals, minerals, vitamins and even natural antibiotics, it is the most popular mud used on spa treatments today.

Volcanic mud. Mixture of volcanic ash and water, with a high concentration of essential trace elements which are thought to be beneficial in drawing out impurities and controlling excess oil, Can also make pores smaller.

Thermal spring mud. Purported to have specific medicinal properties. Sometimes referred to as ”fango” although technically this only applies to mud found at the thermal springs in Battaglia, Italy.

Ocean mud. Taken from the bottom of the ocean floor.

Mud for Medicinal Reasons
Because of its high mineral content, mud has also been used for centuries as a medicinal treatment. It is thought that the mineral content successfully seeps through the skin, enters the bloodstream and thus circulates freely around the body – and also stimulates the nerve endings in our skin to influence the entire nervous system. Proponents say it can cure or relieve a variety of ailments, although skeptics are more than skeptical. They include:

Arthritis

Rheumatism

Joint Pain

Fibromyalgia

Muscular atrophy

Post-surgical complaints

For thousands of years the human race has made the most of natural ingredients to cure our ills, and to make us look better while we’re at it. Mud therapy and chemical peels are no exception: they are simply a modern-day form of pampering the way nature intended.

If you are interested in trying either, you might want to try a mud bath first – although the results may not be as dramatic, it sounds like it would be a lot more fun – and a lot less painful.

At a loss about which med spa treatment, or cosmetic medical procedure, is right for you? From Artecoll to Smart Lipo to Wrinkle Peels, this list should help…

A is for Artecoll. A dermal filler that claims to be “permanent” as the results last five years or more. Can be used to plump out facial wrinkles in particular and, occasionally, in the lips – although it’s not recommended as the result can be disastrous! Make sure you have an allergy test before having this treatment – and the jury is still out about whether it is 100 percent safe.

B is for Botox. This FDA-approved injectable med spa treatment is made from a Botulinum Toxin A, a bacteria that can cause food poisoning. It works by either making us have chronic diarrhea, or paralyzing the muscles that can cause us to frown and make other wrinkles appear. Amazingly, it can also do wonders for migraines, excessive seating and even incontinence!

C is for Cellulite Treatments. Cellulite is just plain old fat, and the best way to get rid of it is through diet and exercise. If that fails, liposuction is one way to stop that orange peel look from taking over, as well as less invasive treatments such as LipoSolve, Smart Lipo and others. Many potions and lotions also claim to work, as well as procedures such as Lipomassage, Velasmooth and a host of rivals.

D is for Dermaplaning. A rather extreme method to get rid of acne scars and other skin problems by having someone scrape the skin off your face with a scalpel. Some people swear by it, others say it’s a bit too brutal. You be the judge.

E is for Elite-Lipo Dissolve. A fat-busting treatment designed to get rid of lard in problem areas, such as love handles. You will have a series of injections to melt the fat away, along with attendant pain and discomfort. Stubborn areas usually respond well.

F is for Foot Implants (no, really). Wanna dance all night? Make the pain of high heels a thing of the past by having a filler injected into the soles of your feet. Try Perlane, Restylane or collagen – incredibly painful while you’re having the injection, but isn’t that what beauty’s all about?

G is for Goretex. While it may sound like a brand of waterproof material, this is used as an non-toxic implant to plump out deep laugh lines, wrinkles and lips. As the body doesn’t reject this substance, results are longer lasting than with other fillers. Also, because it’s flexible with microscopic pores, it’s has a low predisposition toward tearing.

H is for Hair Transplant.”Donor” hair from the back or sides of your scalp are placed in the areas of your head where there is no hair, or where it is very thin. The idea is to have new hair grow in places where it has stopped growing. Painful and has limited results for most people.

I is for IPL Photofacial (Intense Pulsed Light). Combat the signs of aging – and the redness of rosacea – with light energy which stimulates the production of collagen. You will need at least five to ten treatment for maximum effect.

J is for Juvederm. Another filler for lips, face and other body parts. The beauty part, however, is that 80 of the Juvederm injected is safely reabsorbed by the body and eventually disappears, while 20 percent stays in the tissues. Lasts longer than Restylane as well – up to a year.

K is for Keratin used in Brazilian Hair Straightening. Using Keratin, this system is touted as being kinder to hair than other types of “permanent hair straightening” systems. Compared to the YUKO system, it is cheaper, takes less time, and wears away gradually. The downside is it only lasts up to four months. Play it safe by making sure your salon uses a cocktail without formaldehyde.

L is for Laser Hair Removal. Make unwanted hair a thing of the past with this treatment, which will make hair disappear almost permanently after time. The hair is zapped with light from a laser, which disables the hair follicles and stops the hair from growing. Nifty, huh?

M is for Male Waxing. Better, faster and more long-lasting than plucking or tweezing, hair waxing for men is becoming more and more popular. Give your hairy back a break or sample a hair-free chest for yourself – waxing isn’t just for women anymore!

N is for Natural Body Contouring. Give yourself an overhaul with laser therapy to zap cellulite – before it zaps you. Non-invasive with long-lasting results, it purportedly works by changing the shape of the fat cells in your body and remodeling the connective tissue underneath.

O is for Obagi Facial. Cleanse your skin – and turn it blue – with this unique facial using Obagi skincare products. The Blue Peel chemical peel reveals fresher skin in just a week, after the old layers of skin have peeled away. Can also be done on other parts of the body such as the chest, hands and neck.

P is for Power Peel. A way to polish your face through the sanding technique of Microderm, or microdermabrasion. An alternative to a chemical peel, it can help get rid of wrinkles, acne scars, age spots and discoloration on the face.

Q is for Q-Med. Impress your cosmetic practitioner by knowing a bit about Q-Med, the medical company that makes medical implants, including Restylane (see below). Headquartered in Uppsala, Sweden, it also makes the injectable gel Macrolane, which is slowly gaining popularity as an “instant” breast enhancer, although it’s not yet approved for sales in the US.

R is for Restylane. A dermal filler manufactured in Sweden, this popular filler can make skinny lips a thing of the past, and can plump up fine lines and wrinkles. It contains hyaluronic acid, which occurs naturally in the body to create volume. Once injected, it works with the hyaluronic acid already present in the body to create even more volume. Dissolves naturally, and can last least six months or more.

S is for Smart Lipo. An alternative to traditional liposuction, this revolutionary new technique involves making fat “melt” away by using a special laser that zaps problem areas. Is cheaper and has less down time that conventional lipo, but you may have lumpy and bumpy results.

T is for Thread Lift. Also known as a “lunchtime face lift”, this med spa procedure involves making special sutures, using a barbed thread, to lift and therefore reposition the sagging parts of the face, such as the jowls. Cheaper and less invasive than a full face lift, but will have to be redone within a few years to keep up the look.

U is for Ugly Scar and Stretchmark Removal. Four main ways exist to get rid of scarring and stretch marks: chemical peel, dermabrasion, laser surgery and full excision. Remember that having realistic expectations is key: your skin will never revert to its pre-adolescent state, so don’t expect too much!

V is for Vein Therapy. A popular way to get rid of thread veins by injecting a special solution directly into them, causing the lining of the vessel to become irritated. It then begins to swell and stick together, making the vein close. As a result blood can no longer flow through it, so the red or blue colour disappears.

W is for Wrinkle Peel. Make old layers of facial skin peel off with a special solution of acid, as the top layers are the ones most likely to wrinkle. Sounds tempting? Remember that a wrinkle peel can take up to a month to recover from, with risks including infection, scarring, permanent skin lightening and inflammation of the skin.

X is for X-ray. Keep healthy by knowing exactly how your internal health is doing. Some med spas offer X-ray absorptiometry scans, which can check your BMI, or Body Mass Index, as well as your bone density, before you have a treatment. Good to know before following any diet plan – or having lipo!

Y is for Youthful Skin. Skin rejuvenation treatments are perhaps the most sought after procedures offered in med spas today, as virtually everyone wants to look – and – feel younger. Don’t forget, however, that exercise and proper diet go a long way to a healthier new you.

Z is for Zinc. Try this and ditch the skin rejuvenation treatments, as prevention is often better than cure. Keep yourself looking younger by religiously applying sunscreen containing zinc oxide, which acts as a barrier against the sun’s harmful rays – and is also used as a barrier cream for babies with diaper rash!
